The services included, exclusions, timing, location and the owner for each action.

Evidence produced

Supplier records, audit findings, inspection photos, sample observations and commercial comparison inputs.

Decision and follow-up

The buyer's documented decision, corrective actions, unresolved risks and subsequent review date.
